11/2/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival, Client appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C engaged her in a discussion to determine behaviors since the last session. Mother reports she continues to struggle with coping skills at least 3-5 times on a daily basis lasting about 1 hour. 2. Behavior: During the second portion of the session, client was observed by QMHP displaying aggressive behaviors towards her mother such as speaking with her in a high tone of voice (i.e. yealling) at least 2-3 times within a 2 hour period. 1.Intervention: QMHP-C praised and recognized Client for her hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. QMHP-C supported Client with meeting her objectives and coping with stressors without needing additional redirection or prompts. QMHP-C encouraged Client to continue displaying age appropriate behavior, and the benefits of this will be reviewed with parent and other services. QMHP-C conducted a review of previous week regarding mindful listening. QMHP-C assisted Client with problem-solving. 2. Intervention: QMHP-C provided Client with feedback. QMHP-C provided Client with positive reinforcement. QMHP-C educated Client about using good manners in the home and offering alternative methods and suggestions to model appropriate language that Client should use to ask for things or to get someoneâs attention. QMHP-C engaged Client in a role reversal exercise and required Client to think about her goals for the future. 1.Response: Client responded in a positive manner acknowledging the positive reinforcement that QMHP-C provided by saying âthank youâ while presenting a smile. Client utilized relaxation strategies. Client engaged in self-reflection and reflected on her day, the good and the bad. Client communicated her likes and dislikes. Client recognized her triggers and was able to identify common interests and similarities her and other ADHD recipients share. Response: Client was receptive to the interventions, as evidenced by her cheerful expressions. Client demonstrates leadership skills by helping others in the home. Client communicated being home was a positive place for her compared to her out of home environment. Client displayed high ambition and was very productive, despite distractions. Client discussed mental illness and its causes as a family. Client displayed interest in learning about ADHD.
